Step 1

Check that your patient qualifies

Recuperative care is covered by Minnesota Health Care Programs when the patient meets all of the following:

Age 21 or older with Medical Assistance, or 19 or older with MinnesotaCare
Currently experiencing homelessness or otherwise unhoused
Needs short-term medical care and support, expected under 60 days
Independent with activities of daily living

Step 2

Send us the referral

An advanced practice provider (physician, nurse practitioner, or physician assistant) issues discharge instructions, then your team sends us:

Patient name, date of birth, and MHCP member information
Discharge instructions and the current care plan
Medication list and upcoming follow-up appointments
Contact information for the referring care team

Medical eligibility

Confirm the individual meets all required criteria for recuperative care services. (all required)

The individual is 21 years or older (if Medical Assistance) or 19 years or older (if MinnesotaCare).

The individual is experiencing homelessness or is unhoused.

The individual has a short-term medical condition that requires support or monitoring but does not require hospitalization.

The individual can independently complete activities of daily living, including using the bathroom and transferring.

The individual's behavioral health needs do not exceed what can be supported in a community-based setting.

The individual would benefit from supportive services such as:

Common questions

What does it cost the patient?

Nothing out of pocket for eligible members. Recuperative care is a covered benefit under Minnesota Health Care Programs, and we work directly with managed care organizations on reimbursement.

How long can a patient stay?

Stays are available from 21 days up to 60 days, depending on the patient's recovery and housing plan.

What is the facility like?

Dorm-style rooms designated by gender, with 24-hour staff availability, daily wellness checks, three meals a day, secure storage, and telephone access.

Who supervises the care?

Health services are supervised by an enrolled advanced practice provider, with nursing support, wound management, and medication support on site.

What happens at discharge?

Discharge planning starts on day one. We connect each guest to housing resources, case management, and community services, and we share outcomes with the referring team.
